We understand that arriving at a doctor’s office for the first time can be stressful. Filling out paperwork, learning new faces and names, and getting to know your way around the office can be overwhelming.

At Stakes Chiropractic Center, we aim to remove unnecessary tension and make you feel comfortable. Below you can take care of the initial paperwork.  Just print the appropriate new patient form, complete it at home, and bring it to your initial appointment. 

At Stakes Chiropractic Center, we believe quality wellness care should be accessible to all.  This is why we have not changed our prices in 16 years.  While we’re not in-network with insurance plans, we gladly file claims on your behalf.  We will be happy to verify and explain your coverage to you. Please remember, your insurance is a contract between you and your insurance carrier. You are ultimately responsible for your bill.

We do accept auto accident insurance and work closely with your attorney. 

At Stakes Chiropractic Center, we’re convinced that a small amount of your time and money to care for your most important asset—your health—will be far less time-consuming and expensive than trying to recover it.  

So please, we would like to encourage you to take your health very seriously. As the old saying goes, “if you don’t have your health, you don’t have anything.” Unfortunately, too many people don’t discover how true that statement is until it is too late.
